Upon scheduling an appointment and being entered into our electronic medical records system, you should automatically be sent (via email) the required forms to be filled out for your initial visit. If you should not receive the forms for some reason, you may call the center at (205) 599-1020 or access the documents below.

If you are unable to fill out the forms in your browser, download the most recent version of Adobe Reader (Free), NOT PRO (not Free), if you do not already have it. You may click on the Adobe Reader icon below to get it. Once installed, copy the documents in the links below to your computer. You can then print that page and fill it out to bring in to the center with you.

New Patient Packet (pdf)

Please Read These Instructions Carefully: The above New Patient Packet should be opened and then filled out in its entirety in your browser and then printed to your printer. Again, Firefox browser works best for this. If you are unable to fill the forms out in your web browser, you can right click on the “New Patient Packet” file link above (control + click or right click for mac) and save it to your computer that way. Then open it with Adobe Reader, fill it out and print it on your printer. Bring it with you to the center on your upcoming visit.

  • RESPIRONICS RECALL

    If you have a Respironics or Philips Respironics CPAP/BiPAP machine (Respironics is now owned by Philips.), there is a recall affecting some of those machines. For more information about the recall and whether it affects you, please click on the link below to visit their recall page on the internet. You will be able to register your machine with them for addressing the issue.

  • Current Research Studies

    Our center performs Research Studies and Clinical Trials to help determine the effectiveness of new drugs and treatments. We are currently seeking participants for the following studies:

    The Sleep Disorders Center of Alabama offers many different clinical trials for our patients. These are always changing, making it difficult to list on our site. Please call or email Wes Booth at 205-599-1020 extension 122 / wbooth@sleepalabama.com for current trials offered at the Sleep Disorders Center of Alabama.

    Please be aware that CPAP equipment wears out. You should replace your mask, headgear and hoses about every 6 months. Please call the Sleep Center’s CPAP Department at (205) 599-1020
